Outpatient:

We provide one-on-one physical, occupational, and speech therapy services for patients of all ages. Our goal is to get you better faster so that you can get back to enjoying the activities Jackson and Teton Valley have to offer. Our team of experienced therapists will help you develop an individualized plan of care to enhance your recovery and experience.

Acute Care:

Acute care rehabilitation at St John’s Medical Center consists of therapy services provided in a hospital setting by physical therapists, occupational therapists, and speech therapists. Therapists coordinate care with physicians, nurses, social workers, and case managers to optimize patient care and rehabilitation. Our therapists specialize in wound care, swallow studies, cognition, adaptive equipment training, mobility training, therapeutic exercises, and improving patient’s ability to perform activities of daily living. Therapists also work closely with St. John’s Peak Spine and Joint Replacement Program to enhance outcomes of surgeries. All therapy services are focused on assisting patients to reach their full physical, cognitive, and functional potential. Patients who experience any functional impairment or physical deconditioning during their hospital stay may benefit from acute care rehabilitation

Long Term Care:

St. John’s Living Center is a 60-bed skilled nursing facility which strives to provide the highest level of care to all residents. As an Eden Alternative Certified home, The Living Center minimizes helplessness, hopelessness, and boredom through engaging events, programs, and activities. The Living Center was recently recognized by the US News and World Report as one of the top nursing homes in the country, specifically “High Performing” for short-stay rehabilitation and long-term care. The Living Center is able to provide residents with physical, occupational, and speech therapy 7 days/week. All therapists in the Living Center are specialized in treatment of the geriatric population. Rehabilitation in this setting utilizes a team approach, with input from physicians, nursing staff, social work, life enrichment, and all therapy disciplines to help residents achieve their goals.

Home Health and Hospice:

Teton Physical Therapy and Rehabilitation works to provide interdisciplinary rehabilitation services for both home health and hospice departments. Our team is comprised of speech language pathologists, occupational therapists, and physical therapists. We work closely with the home health and hospice nurses, CNAs, and social workers. Home health and hospice services are provided for patients who are medically unable to leave their homes to receive care. We are able to address all ages and a wide spectrum of diagnoses that include, but are not limited to, neuro-progressive diseases, significant orthopedic traumas, spinal cord injuries and traumatic brain injuries, cognitive changes, swallowing difficulties, lymphedema, and wounds. We also perform home safety evaluations and recommend appropriate adaptive equipment to improve patient safety and function.
